Allegheny County Level of Care Descriptions (from the pilot housing referral)

CRR Apartment (Melwood, Penn, and Stanton)

♦ Is 18 years of age or older;
♦  Has a primary diagnosis of a serious mental illness
♦  Requires 24/7 support in a short term Community Residential Rehabilitation facility with a stay of upto
12 months and agreeable to living in a single or shared apartment with the possibility of sharing common
living areas such as bedroom, bathroom, and kitchen.
♦  is capable of living independently but requires moderate assistance with Activities of Daily Living Skills
such as cooking and cleaning with the goal of moving to more permanent housing.
♦  Requires structured activities that will require the individual spending a majority of their time outside of the CRR.
♦  May require medication monitoring by staff with the goal of being self medicating.

 

CRR Group Home(Balph, Millvale, Hasina, and Maryland)

♦ Is 18 years of age or older;
♦  Has a primary diagnosis of a serious mental illness
♦  Requires 24/7 support in a short term Community Residential Rehabilitation facility with a stay of upto
12 months and agreeable to living in a single or shared apartment with the possibility of sharing common
living areas such as bedroom, bathroom, and kitchen.
♦  is unable of living independently at this time and requires significant assistance with Activities of Daily
Living Skills    such as cooking and cleaning with the goal of moving to more permanent housing.
♦  Requires structured activities that will require the individual spending a majority of their time outside of the CRR.
♦  May require medication monitoring by staff with the goal of being self medicating.

 

MISA CRR-Group Home (South Aiken)

♦ Is 18 years of age or older;
♦  Has a primary diagnosis of a serious mental illness and substance use disorder;
♦  Requires 24/7 support in a short-term highly structured Community Residential Rehabilitation facility with
the stay of upto 12 months and is agreeable to living in a single or shared apartment with the possibility of
sharing common living spaces such as bedroom, bathroom, and kitchen.
♦ Unable to live independently at this time, and requires significant assistance with Activities of Daily Living Skills
such as cooking, cleaning with the goal of moving to the next level of care;
♦  Requires structured activities that will require the individual spending a majority of time outside the CRR.
♦  May require medication monitoring by staff with the goal of being self medicating;

 

Specialized Residence  (Wetzel)

♦ Is 18 years of age or older;
♦  Has a primary diagnosis of a serious mental illness and, in some cases,  have a secondary diagnosis of
personality disorder;
♦  Requires 24/7 support in a short-term or long-term highly structured 3 person specialized structured
residence.  Agreeable to living in a single or shared apartment with the possibility of sharing a bedroom,
bathroom, and  kitchen.
♦  Has complex needs and is unable to live independently or in any other residential facility at this time.
Requires significant assistance with Activities of Daily living skills such as cooking and cleaning
♦   Has the potential toward working on short and long term goals and with the objective of moving to a lower level of
care.
♦  Ready for discharge from a mental health facility such as and Extended Acute Unit (EAC), Long Term Structured
Residence(LTSR) ; Residential Treatment Facility for Adults(RTF-A) , Transitional Recovery Unit(TRU), or
Comprehensive Recovery Unit (CRU), or community hospital.
♦  Requires medication administration by staff with the goal of self medicating.
